Licensed Marriage and Family Therapist

Role: Licensed Marriage and Family Therapist (LMFT) providing in-person psychotherapy to geriatric patients in skilled nursing, sub-acute, long-term care, and assisted living facilities.

Responsibilities Include

• Conducting individual and group therapy sessions
• Assessing, diagnosing, and treating emotional and behavioral health conditions such as depression, anxiety, and cognitive decline
• Developing and implementing evidence-based, patient-centered treatment plans
• Collaborating with facility staff, interdisciplinary teams, and families
• Maintaining accurate clinical documentation in compliance with regulations
• Monitoring patient progress and adjusting treatment as needed
  
  

Qualifications And Skills

• Active, unrestricted LMFT license in the applicable state
• Effective communication and organizational skills
• Proficiency with electronic health records (EHR)
• Ability to provide in-person services within facilities
• Full vaccination and booster may be required in some facilities
