- Conduct initial and ongoing assessments of mobility, strength, and function.
- Administer, evaluate, and interpret tests to set and track functional goals.
- Provide written evaluations and care plans outlining needs, goals, and progress.
- Deliver physical therapy services according to physician orders.
- Maintain documentation, modify treatment plans, and plan discharges.
- Collaborate with healthcare teams and educate patients/caregivers on home exercises.
- Ensure compliance with policies, regulations, and documentation standards.
- Supervise other personnel such as PTA or HHA when applicable.
- Complete assessments like OASIS where permitted by regulations.
- Degree in Physical Therapy from an accredited program.
- Current, unrestricted state license.
- Proficiency in assessments, documentation, and compliance.
- Valid CPR, driver’s license, and automobile insurance.
- Experience in home health therapy and OASIS documentation preferred.
