Job Description
Qualifications: Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care. Must be a Registered Nurse in good standing and currently licensed by the State; must be able to work after hours and weekends as needed.
Education: Graduate of an accredited school of nursing, BSN preferred.
Licenses/Certifications: Valid California RN license, Valid CPR certification
Experience Required: Previous experience in a hospital, long-term facility, or other healthcare related facility preferred. Previous experience in completing MDS, RAI process and Care Plans is required. Experience as an MDS Coordinator and knowledge of RUGS optimization is important.
Essential Functions:
- Participate in peer support / evaluation, in services, education programs, and staff meetings, and Quality Improvement activities as assigned.
- Schedule and obtain accurate resident information to transmit MDS assessments electronically in compliance with Regulatory/Medicare requirements and CMS guidelines.
- Ensure the timely electronic submission of face validity of all Minimum Data Sets to the state database.
- Review the state validation reports and ensures that appropriate follow-up action is taken.
- Schedule and meet routinely with residents, families and staff to meet resident/family short and long-term treatment goals.
- Create an opportunity for family participation in the care planning process and develop collaborative treatment plans.
- Perform assessment of residents regarding changes of condition and develop appropriate comprehensive resident assessment and care plan.
- Coordinate the development and completion of the resident assessment (MDS) in accordance with all Federal and State guidelines.
- Select the correct (ARD) Assessment reference date (ARD) and correct reasons for assessment that capture all resources to ensure appropriate payment by Medicare/Medicate systems.
- Obtain Quality Indicator reports from CMS, report findings including strengths/weaknesses identified and discuss strategies with department heads for maintaining quality care for residents.
- Collaborate with Nursing, Therapy, Dietary, Social Services and Activities to maximize treatment efficacy to meet long/short term resident goals.
- Any other duties as assigned by supervisor

- Must possess strong clinical skills.
- Ability to communicate effectively with residents and family members.
- Ability to react decisively and quickly in emergency situations.
- Ability to prioritize and meet MDS assessment submittal deadlines.
- Knowledge of training techniques for clinical staff.
- Ability to maintain confidentiality at all times.

procedures regarding nursing standards and delivery of care.
