Job Title: Client Support Coordinator

Location: Alameda, CA (In-Person, On-site)
Employment Type: Part Time (Weekends and Evenings)
Compensation: $24.50 to $29/Hr
About Us: The Village of Love Foundation is a nonprofit organization dedicated to serving the homeless population in Alameda County. Our mission is to provide comprehensive support, resources, and advocacy to individuals and families experiencing homelessness, helping them regain stability and independence.
Job Duties & Responsibilities:
- Work with clients and health care professionals to connect clients to services
- Listen to clients’ concerns and referring them to counseling or intervention as required
- Record clients’ progress in HMIS and charting referrals and other notable interactions
- Evaluating clients’ progress periodically and making adjustments as needed to improve outcomes
- Refer treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation
- Maintains clients’ records by reviewing case notes; logging events and progress.
Skills & Qualifications:
- Compassionate and able to relate to different clients with various needs
- Strong verbal and written communication skills
- Critical thinking and problem solving to determine the best care plan for each client after assessing clients, analyzing notes from health care and social workers
- Organization to manage several different cases at once
- Computer literacy to maintain and manage case records
Education and Experience:
- Bachelor's degree in Human Services, Social Work, Public Health or Health Administration is preferred.
- In lieu of degree, 4 years customer service experience, preferably in human services, possess demonstrated commitment to customer service.
- Proficient in computer systems and G-Suite applications.
- Professional experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferred
- Must have a valid in-state driver’s license, provide current registration and insurance, and have a driving record with no more than 2 points within the last 3 years
- Must be over 21 years old

- Saturdays and Sundays: 8:30 AM – 5:30 PM
- Saturdays and Sundays: 12:00 PM – 9:00 PM
- On-Call availability as well.
