Job Description
POSITION SUMMARY:
Prescribes and adheres to the Villa Way of Kindness in all personal and professional interactions while on
duty in and around the facility. Director of Social Services – Licensed demonstrates great customer
service by mentoring Villa’s three core principles of Exceptional Care, Passionate Service and
Unexpected Luxury in all of his\her job duties and responsibilities.
The Director of Social Services - Licensed is responsible for providing social services to patients
of the comprehensive outpatient rehabilitation program. They evaluate patients and identify need
areas by coordinating and directing other social workers in carrying out their job duties. Additionally,
coordinates services with the interdisciplinary team and contacts community resources as needed.
The Director of Social Services - Licensed oversees case management services for patients and
reviews patient status. The position is supervised directly by the Administrator and does
administratively supervise other positions when specified per center. Strictly follows policy and
safeguards computer passwords and computer use authorities. Understands the importance of logging
on and off the computer clinical record system and will consistently maintain all computer protocols.
Administrative Workers
❒ Perform office or non-manual work directly related to the management policies or general
business operations of their employer or their employer’s customers, or perform administrative
functions directly related to academic instruction or training in an educational establishment
❒ Regularly exercise discretion and independent judgment with respect to matters of significance
❒ Are compensated on a salary basis at a rate at least $455 per week
REPORTING RELATIONSHIPS:
Assistant social worker where applicable based on facility needs
ESSENTIAL FUNCTIONS:
1. Evaluate patient need for social services.
2. Coordinate social services with patient, staff, family members, and significant others as applicable.
3. Provide social services as specified in the patient plan of care and consistent with treatment goals.
4. Perform case management or case coordination as needed for patients.
5. Contact and coordinate patient utilization of community resources.
6. Provide social services to patients in accordance with accepted standards of social work and
all applicable federal, state, and local laws and regulations and interpretations thereof.
7. Provide specialized training to other staff, family members, consultants, and other individuals
as needed.
8. Participate in clinical case conferences and in-service programs to share information with
the interdisciplinary team.
9. Maintain appropriate documentation in a prompt and comprehensive manner.
10. Attend team meetings and other meetings as required.

Qualifications
MINIMUM QUALIFICATIONS:
Education: Minimum of a Bachelor Degree in social work or social welfare from an accredited
academic institution. Preferable a Masters. Possession of a current state license as a Social Worker.
Experience: Experience in outpatient medical rehabilitation preferred.
Physical Demands:
Primary activities: involve driving, walking and talking. Involves sitting and standing.
Travel: Up to 90%
Working conditions: Primarily inside with well- lighted and well ventilated areas.
Skills, Knowledge and Abilities: Ability to plan and adapt policies and procedures relating to social
services. Strong organizational skills. Demonstrate integrity, imagination and initiative, showing
appropriate manner and conduct. Ability to relate positively, effectively and appropriately with residents,
families, community members, volunteers and other facility staff. Possess special interest in, and a
positive attitude about, working with long-term care residents and the elderly. Ability to read,writes, speak, and understand English. Meets all health requirements, as required by law. Demonstrates
basic computer knowledge and ability with an aptitude to learn company applications.
