Responsibilities
- The Medical Social Worker, MSW, provides direct discharge planning services and assistance with psychosocial services for the patient and family.
- Collaborates with physicians and other members of the healthcare team to develop a discharge plan in relation to the patient's health care needs.
- Utilizes broad experience and knowledge of appropriate healthcare resources for patients of all ages.
- Promotes continuity through communication of the patient's plan of care.
- Reports to the Clinical Resource Management Coordinator.
Qualifications
Job Requirements:
- Master of Social Work degree.
- Kansas Licensure at the LMSW level.
Preferred Job Requirements:
- Two semesters of graduate field experience in healthcare setting for a new graduate or previous experience in healthcare setting for an experienced graduate.
