Lead Case Manager – SSVF Program
Full-Time | Non-Exempt | Local Travel Required

We are seeking an experienced, housing-focused professional to serve as Lead Case Manager within our Supportive Services for Veteran Families (SSVF) program. This is a working lead role responsible for providing day-to-day oversight of case management operations while maintaining strong compliance, documentation integrity, and performance accountability. You will help oversee and support a team of case managers serving veterans and their families experiencing homelessness or housing instability. This role ensures adherence to VA and SSVF regulations, strengthens HMIS quality, supports housing placement efforts, and reinforces a Housing First, trauma-informed service model. If you are a detail-oriented leader who understands veteran systems, housing stabilization, audit readiness, and team development — this position offers the opportunity to elevate both outcomes and staff performance.
This position reports to the Program Manager and provides functional supervision to assigned Case Managers.
What You’ll Be Responsible For:
Program & Team Leadership
- Oversee daily operations of the SSVF case management team
- Provide coaching and workflow oversight to case managers
- Contribute to performance evaluations in coordination with the Program Manager
- Facilitate staff meetings and document meeting minutes
- Model professional standards and reinforce accountability
Compliance & Documentation Integrity
- Ensure adherence to SSVF regulations and VA guidelines
- Maintain HMIS documentation accuracy and audit-ready files
- Review client files for completeness, compliance, and quality assurance
- Complete required Squares Account Manager training and Veteran eligibility determinations
- Monitor HUD Self-Sufficiency Matrix updates
- Submit and track Temporary Financial Assistance (TFA) requests with proper budget documentation
Veteran Housing & Stability Oversight
- Coordinate assessment of client eligibility for VA benefits in collaboration with VA representatives
- Track VA applications and ensure timely documentation submission
- Monitor client progress toward housing stability, employment, and self-sufficiency goals
- Support housing placement activities including landlord coordination, inspections, and housing counseling
- Reinforce Housing First and trauma-informed care practices
What You Bring:
Required Qualifications
- Bachelor’s Degree in Social Work, Human Services, Psychology, or related field (Master’s preferred)
- 3–5 years of progressively responsible case management experience
- Experience serving individuals and families experiencing homelessness, including those with substance use or mental health disorders
- Working knowledge of VA benefits systems and eligibility processes
- Strong HMIS documentation proficiency
- Valid driver’s license, dependable vehicle, and current auto insurance
- Ability to maintain confidentiality and exercise sound judgment in crisis situations
Preferred
- Experience in SSVF or other VA-funded programs
- Supervisory or team lead experience
- Familiarity with Housing First and trauma-informed care models
- Bilingual (Spanish/English)
- Audit preparation or monitoring experience
Work Environment & Schedule
Full-time position aligned with program operations. Occasional evening flexibility may be required to accommodate client needs, audits, inspections, or financial deadlines. Frequent local travel to client homes, housing sites, and community partners is required. Must maintain a valid driver’s license with clean driving history and current auto insurance. This role includes both office-based documentation work and field-based housing inspections and client engagement.
