The Sisters of St. Francis of Philadelphia are a community of Catholic women who have committed their lives to God as followers of Saints Francis and Clare.
Position Summary:
We operate Assisi House, a 125-bed multi-level care facility for retired sisters in need of medical support. We have an exciting opportunity for a Wellness Services Coordinator to join our Wellness Services Department.
The Coordinator will focus on addressing the wellness needs of sisters who are living independently (or independently with some services) on campus, but who do not require the more advanced services and medical care provided for sisters living on the medical units within Assisi House.
The Wellness Services Coordinator assists the Director of Wellness Services by supporting our sisters’ health and wellness, and by coordinating health and wellness education. The Coordinator provides care management services related to our sisters’ physical and cognitive health needs, and participates in a 24-hour on-call rotation.
Essential Functions:
- Exemplify the values and charism of the Sisters of St. Francis of Philadelphia.
- Understand health issues of sisters.
- Listen to sisters’ needs with care and compassion.
- Assist sisters in making personal healthcare plans as requested.
- Coordinate care for sisters who have complex healthcare needs, such as diabetes, heart, or pulmonary disease to prevent complications and promote wellness.
- Communicate with physicians, nursing/social work, PT/OT and other medical supportive staff with the goal of understanding and implementing an optimal health care plan.
- Assist sisters with scheduling health provider/diagnostic appointments as needed.
- Drive sisters to healthcare appointments, if needed.
- Monitor vital signs such as blood pressure, temperature, pulse ox, and heart rate to track the health status of sisters and report any changes to the Director of Wellness Services.
- Assess sisters’ mental status and cognitive ability, and alert Director of Wellness Services if further testing is recommended.
- Assess sisters’ personal hygiene and ability to perform Activities of Daily Living (ADLs).
- Assist sisters with medication management and organization as prescribed by their health care providers to ensure proper dosage and timing, as needed.
- Respond to emergencies and provide first aid and/or call 911 to ensure the safety and well-being of the sisters.
- Maintain healthcare records for sisters and develop reports as needed to provide details and summaries for review by the Wellness Services team and/or medical specialists.
- Participate in a 24 hour on-call rotation for the emergent needs of sisters.
- Perform other duties and assignments as requested by the Director of Wellness Services within the general scope of the Wellness Services Department function.
- Maintain knowledge of best practices and remain current with new healthcare trends, research, and recommendations.
Qualifications:
- 5 years professional experience in healthcare/wellness. LPN or other healthcare license/certification is a plus.
- 3 years of geriatric care experience.
- Good time management skills.
- Strong organization skills.
- Outstanding interpersonal skills.
- Excellent verbal and written communication skills including the ability to adapt messaging to the audience (i.e. sisters, family/friends and healthcare professionals).
- Ability to work collaboratively as a contributing member of a team.
- Strong conviction to maintain confidentiality and honoring privacy.
- Adept at establishing and maintaining professional boundaries
- Compassionate and sensitive to people’s needs, with a demonstrated desire to be of service.
- Good listening and consultative skills.
- Ability to maintain calm demeanor during medical emergencies including clear-thinking and good decision-making.
- Proficient at problem-solving and the ability to sound judgment.
- Current Driver’s License with a clean driving record.
- Proficient with Microsoft Outlook, Word, and Excel.
- Flexibility to perform on-call rotation for evening and weekend coverage.
Physical Requirements
- Tolerate long periods of standing, walking, bending
- Move around throughout the day, within the primary office building and Assisi House as needed.
- Ability to help sisters get in and out of vehicles if they need physical support.
