Qualified Professional (QP) – Community Support Team (CST)

We are building a high-standard CST program in rural North Carolina. Our clients often face serious barriers: mental health symptoms, substance use, housing instability, justice involvement, limited transportation, and inconsistent phone access.
We’ve learned the hard way that CST fails when staff:
- Don’t want to drive and see clients in person,
- Don’t follow up consistently,
- Avoid documentation (especially NC TOPPS),
- And don’t help grow the program.
So we’re being clear up front: this role is for a builder—a QP who wants to help strengthen the program, support leadership, and actively grow the client base through outreach and partnerships.

What you will be responsible for:
- 1) Field-Based Client Support (Weekly Contact Required):
- You will meet with clients in person at least weekly (more when risk is elevated). Visits may occur in homes, community settings, shelters, DSS/hospital settings, probation/parole-approved locations, or other safe meeting points.
- 2) Crisis Support (Rotating On-Call):
- You will participate in rotating on-call crisis phone support with clear escalation protocols and supervisor backup.
- 3) Person-Centered Plans (PCPs) Clinical Oversight:
- You will develop, update, and drive Person-Centered Plans (PCPs) that are specific, measurable, and tied to real outcomes—not generic templates.
- 4) Care Coordination (Closed-Loop Follow-Through):
- You will coordinate psychiatric evaluations, medication supports, therapy, housing resources, and community services—and follow through until outcomes are confirmed. We don’t do “resource lists.” We do results.
- 5) Documentation & Compliance (Non-Negotiable):

Documentation is not optional here. You will complete timely, accurate documentation aligned with CST standards, including:
- NC TOPPS documentation (completed correctly and on time)
- service notes, PCP updates, care coordination logs
- documented outreach attempts and engagement efforts
We provide templates and training, but we hold a high standard for quality and timeliness.
Program growth is part of the job (this is what separates us)
This role is not only about serving the current caseload. You will also help build the pipeline.
Outreach & Client Engagement:
You will support outreach efforts to increase client enrollment and reduce fall-off by:
- Following up quickly on referrals and inquiries
- Re-engaging clients who miss appointments or go “quiet.” Helping clients overcome barriers (transportation, phone access, motivation, instability)
Partnership Support (Referral Relationships):
You will support partnership-building and referral relationships by:
- Helping maintain contact with key referral sources (MCOs, DSS, probation/parole, hospitals, shelters, community agencies)
- Participating in community outreach events when needed
- Sharing feedback with leadership on what partners need to refer clients successfully Helping the team become known and trusted across rural counties

Team expectations (high standard)
Weekly Team Meetings Are Required:
You will attend weekly CST/team meetings prepared with updates, barriers, and next steps. These meetings are how we keep clients safe, prevent missed follow-ups, and keep the program strong.
Support Team Leads Step Up When Needed:
You will support team leads and step up when:
- a lead is out
- Coverage is needed for follow-up The program needs stability and leadership support
“Hard-to-locate” clients: what we expect
In rural CST, some clients will be difficult to reach. We need a QP who is persistent and thorough, using ethical outreach practices consistent with policy, such as:
- Multiple contact attempts at different times/days
- Coordination with referral sources when appropriate
- Contacting authorized support with releases
- Safe community engagement strategies consistent with policy Documenting every attempt and outcome clearly
This is not “extra.” This is rural CST done correctly.
Compensation Incentives:
- $22–$25 per billable hour (based on experience)
- Administrative time: 6 hours/week at NC state minimum wage
- Mileage allowance: $60/week

Performance Bonus Opportunity: Available for QPs who consistently meet standards and help grow the program, including:
- Documentation compliance (including NC TOPPS timeliness/accuracy)
- Consistent weekly contact follow-through
- Outreach/referral conversion (helping bring clients into active services) Partnership support and retention metrics
Bonus structure will be discussed during the interview and aligned with measurable performance targets.
Qualifications (QP Requirements):
- Master’s in Human Services 1 year relevant experience, OR
- Bachelor’s in Human Services 2 years relevant experience, OR Bachelor’s in Non-Human Services 4 years relevant MH/SUD experience
Reliable transportation is required.
