Housing Case Manager Shelter Alliance

Position Summary: The Housing Case Manager is responsible for supporting individuals and families experiencing homelessness or housing instability through assessment, service planning, housing navigation, advocacy, and coordinated referrals. The position requires strong documentation practices, professional judgment, and collaboration with community partners to promote participant stability and long-term housing success.

Key Responsibilities

  • Conduct participant intakes, assessments, face-to-face and phone contacts, and ongoing follow-up.
  • Develop and maintain individualized life plans in collaboration with participants to address:
  • Housing instability
  • Food insecurity
  • Income and employment pathways
  • Healthcare and behavioral health needs
  • Transportation and other essential supports as needed
  • Provide trauma-informed, participant-centered case management and advocacy
  • Assist participants in obtaining state and federal documentation and benefits, which may include birth certificates, state identification, medical assistance, Electronic Benefit Transfer (EBT) cards, and related supports.
  • Support participants through housing-related processes, including:
  • Emergency shelter placements and transitional housing referrals
  • Permanent housing navigation
  • Coordinate referrals and communication with partner agencies, service providers, landlords, and community resources
  • Monitor participant progress and adjust service plans as needed
  • Attends Mid Shore Roundtable on Homelessness HCP/CE monthly meeting (via Teams every month and face to face every three months) to provide the organization with up-to-date Kent County, Maryland, homeless information.
  • Is knowledgeable and able to access Maryland Medicaid to review eligibility of clients
  • Maintain accurate, timely, and compliant case records, including weekly updates to the Executive Director and Board of Directors as requested.
  • Uphold ethical standards, confidentiality, and professional boundaries in all participant interactions
  • Attends meetings and Shelter Alliance functions as required
  • Adheres to all Shelter Alliance policies, procedures, and bylaws.
  • May be required to perform additional duties consistent with the employee’s skills, qualifications, and organizational needs.

Education/Experience

  • Bachelor’s degree
  • Five years of experience in case management or an equivalent combination of education and experience.
  • Must have an in-depth understanding of Homeless Management Information System (HMIS) reporting requirements and be a certified HMIS user in good standing to support compliance with Housing and Urban Development (HUD) standards and guidelines. All information entered in HMIS must meet filing deadlines and be maintained in audit-ready compliance. Initial and ongoing training will be provided as needed.

Working Conditions

  • Work is performed in general office settings and field or community-based locations. The position may require travel to varied locations and flexibility in work hours based on program needs.

Physical Standards

  • Must be able to drive a motor vehicle and tolerate exposure to local weather conditions. While performing the duties of this job, the employee is regularly required to sit, talk, hear, use hands to handle objects, tools, or controls, reach with hands and arms, and stoop. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus. The employee must be capable of traveling on short notice by automobile or other means of transportation and must be able to use standard phone, cell phone, and office equipment.

General Requirements

  • Must be at least 21 years of age and possess a valid driver’s license. Must present and maintain documentation showing that all required credentials are current and valid. Must submit to criminal background and driving record checks from appropriate regulatory and law enforcement authorities. When required by the Shelter Alliance, must submit medical clearance documentation and Tuberculosis (TB) test results, as well as Hep C immunization.

Supervision & Reporting

  • Reports directly to the Executive Director and Board of Directors when necessary
  • Collaborates with administrative staff.
  • Acts in accordance with established guidelines for ethical conduct. Maintains an attitude of cooperativeness, flexibility, and professionalism. Acts within time frames and at quality levels as expected and/or directed by supervisor or Board of Directors. Is amenable to supervision.
  • Opportunity to supervise other staff as the Shelter Alliance evolves over time.

Role Alignment

This role supports The Shelter Alliance’s mission by ensuring consistent, trauma-informed care for participants while collaborating with The Shelter Alliance Board of Directors and executive leadership to focus on organizational oversight, partnerships, fundraising, and facility readiness.

Compensation

  • Base Salary: $48,000–$58,000 annually
  • Bonus up to 5%
  • Benefits: Medical, dental, 401K, consistent with organizational benefits structure
