Position Overview:
The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes, ensuring quality, continuity, and compliance with physician orders and agency standards. The RN is responsible for assessing patient needs, developing care plans, and coordinating with the interdisciplinary team to promote optimal patient outcomes.
Key Responsibilities:
- Perform comprehensive patient assessments (initial, resumption, and ongoing visits)
- Develop, implement, and update individualized care plans
- Administer medications, treatments, and wound care as ordered
- Monitor patient progress and report changes to physicians and care team
- Provide patient and caregiver education on disease management and care plans
- Complete accurate and timely documentation in EMR (e.g., Kinnser EMR)
- Ensure compliance with Medicare/Medicaid guidelines and agency policies
- Coordinate care with therapists, aides, and other healthcare professionals
- Participate in case conferences and quality improvement initiatives
Requirements:
- Active Florida Registered Nurse (RN) license
- Minimum 1 year of nursing experience (home health experience preferred)
- Knowledge of Medicare home health regulations and documentation standards
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage a flexible schedule
- Reliable transportation, valid driver’s license, and auto insurance
- Current CPR certification
Preferred Qualifications:
- Experience with OASIS documentation
- Familiarity with home health EMR systems (e.g., Kinnser EMR)
Pay: $55.00 - $60.00 per hour
