POSITION DESCRIPTIONPosition: Case Manager | Full-time | Benefits Eligible | 35 hours per weekFLSA Status: Non ExemptHours: Monday through Friday, 8:30 am to 4:30 pm/10 am to 6 pm (Subject to change)Department/Location: Domestic Violence Shelter | East Hampton, NY |Remote work is not available for this positionPURPOSETo ready individuals transitioning from an emergency shelter setting to independent living within the community. Ensure that clients' needs are being met through partnerships with counseling and legal advocacy.OVERALL RESPONSIBILITIES● Conduct client assessment, goal planning, and case management for individuals seeking to transition from an emergency shelter setting to independent living● Assist clients with goal planning and conduct regular goal reviews, provide emotional support, assist in connecting clients with available community resources, and develop and maintain interagency relationships● Complete documentation of client contacts appropriately and provide monthly statistical informationSPECIFIC RESPONSIBILITIESClient Interaction● Conduct an intake interview with all new clients arriving at the shelter and complete all relevant paperwork● Complete new client assessments and develop a plan moving toward self-sufficiency, including assessment of clients' need for community-based support● Conduct weekly individual client meetings; regularly review client files● Maintain caseload; document all services provided with accurate, up-to-date notes in the appropriate format● Provide information and referrals to appropriate services● Provide assistance and advocacy appropriate to a client's level of need in obtaining such services● Provide crisis intervention as needed● Review individual incident reports with clients● Arrange medical care and follow-upCommunication● Submit client services and notes to the Apricot database system and submit DVL statistical information to the Suffolk County Department of Social Services● Facilitate and coordinate weekly team meetings● Maintain communication with other staffHotline Management● Answer crisis hotline calls; process proper paperwork for all calls received; enter information into the Apricot databaseOther● Participate in bi-weekly team meetings● Maintain knowledge of and working relationships with community resources and social service providers for housing, DSS, medical, dental, and other local referrals● Attend training/workshops/conferences related to Case ManagementORGANIZATIONAL RELATIONSHIPS● Reports to Shelter Director● The incumbent in this position does not have direct reports but may assist or train othersQUALIFICATIONSBachelor's degree in Social Work or related field and 2 years of experience in any of the following areas: social work, women's studies, psychology, or counseling, or an equivalent combination of education and experience sufficient to perform the duties of the position. Must earn Domestic Violence Liaison (DVL) certification within six months of employment.Required capabilities:● Excellent interpersonal skills; demonstrated dependability and initiative; able to handle crises with minimal supervision● Must maintain program/client/agency confidentiality and treat each individual with dignity and respect● Ability to set and observe appropriate personal limits and boundaries, especially with respect to shelter clients● Flexibility to deal effectively with a variety of people, situations, problems, and changes● Ability to write clear, accurate notes and complete other paperwork thoroughly and accurately● Must be fluent in Word, Excel, and Google DocsOther requirements:● Must have a valid New York State Driver's License (Required)● Bilingual / Spanish (Required)WORKING CONDITIONS● Sits, stands, stoops, squats, walks, and moves intermittently during work hours● May lift, push or pull 10-15 pounds daily; may push or pull up to 50 pounds occasionally● May be subject to hostile and emotionally upset clients● Attends and participates in continuing education programs● Subject to frequent interruptions● All Shelter staff must go up and down stairs carrying donations, including food and clothesOTHER DUTIESEvery effort has been made to make agency job descriptions as complete as possible.
