What are the responsibilities and job description for the Case Manager for Family Homeless Shelter position at Penates Inc?
Full-time Shelter Case Management position available at our family shelter in Bay Shore, NY. The schedule is Monday-Friday, 8am-4pm. A Bachelor's Degree in a related social/human service field is required.
Case manager duties include but are not limited to assessing, planning, implementing, monitoring, and evaluating actions required to maintain clients' DSS case open status. Familiarity with homeless populations and the Suffolk County Department of Social Services procedures a must. Familiarity with HMIS is a plus. Please be a team player!
The Case Manager's job is to work collaboratively with shelter staff and families in the shelter to identify their barriers to independent living, leverage their strengths, work together to overcome these obstacles through direct service or referrals and assist them in moving to permanent housing as soon as possible. Case Managers are on the front line in providing services to clients. A Case Manager’s job is to set clear, measurable goals in an effort to work towards housing permanency. Case Managers empower clients to achieve these goals by creating a collaborative and supportive working relationship with clients based on empathy, accountability and trust.
