Patient Care Coordinator (PCC)

Full-Time | Reports to: Pharmacy Manager

As a Patient Care Coordinator, you’ll be at the heart of our pharmacy operations — ensuring each patient receives timely, personalized support throughout their treatment journey.


What You’ll Do

  • Serve as the primary point of contact for patients, physicians, and care teams regarding prescriptions, deliveries, and pharmacy services.
  • Coordinate orders, verify information, and communicate proactively to keep patients informed and supported.
  • Address questions, concerns, or changes in patient status with professionalism and care.
  • Collaborate closely with pharmacists to ensure seamless care and exceptional service quality.
  • Document all communications accurately and escalate urgent or sensitive issues as needed.
  • Uphold the highest standards of accuracy, compliance, and patient confidentiality.


What You Bring

  • At least 2 years of experience in a medical or pharmacy setting (call center experience preferred).
  • Strong multitasking, communication, and customer service skills.
  • A genuine commitment to improving patients’ experiences and outcomes.
  • Pharmacy Technician License preferred but not required.
