Responsibilities:
- Assess each patient and develop an appropriate treatment plan with measurable goals and time frame
- Prepare and submit clinical and progress summaries based on identified and attained goals
- Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient
- Open/Supervise cases for LPTA's to do follow-ups
Qualifications:
- Active License of Physical Therapy in the state of Virginia
- Prior Homecare experience (preferred)
- EMR system: Kinnser Wellsky (preferred)
