UVA Health is seeking an experienced Registered Nurse Care Coordinator to help advance one ofUVA's newest and most rapidly growing specialty programs: theEhlers-Danlos Center. This is a rare opportunity to help shape a developing multidisciplinary clinic designed for patients with Ehlers-Danlos Syndrome, related hypermobility disorders, vascular EDS, and complex multisystem chronic conditions.

This RNCC role is central to howpatients'access, navigate, and experience care. As the clinic continues to expand, the RN Care Coordinator will serve as a key connector across providers, specialties, patients, and families, helping ensure that each patient's journey from referral through follow-up is organized, responsive, and clinically informed.

The Role

The Registered Nurse Care Coordinator serves as thecentral clinical coordinatorfor a growing specialty clinic that supports adolescents and adults with complex, often lifelong conditions. This role requires strong assessment skills, exceptional organization, and the ability to coordinate across multiple disciplines in a fast-movingambulatory environment.

The schedule isMonday through Friday, 8:00 AM to 4:30 PM, withno weekends, no holidays, and no call responsibilities. This position is currentlyon-site five days per week.

On anaverage day, you may be reviewing incoming referrals, gathering outside records, triaging patient calls, preparing the care team for upcoming visits, and helping patients and families understandthe nextsteps in their care. You will work closely with attending physicians, nurse practitioners, an LPN, access team members, a registered dietitian, behavioral health specialist, genetic counselor, and specialty partners across UVA Health.

Key responsibilities include:

  • Managing referral intake and care coordinationfrom initial referral through ongoing follow-up.

  • Reviewing incoming referralsand ensuring required documentation and outside medical records are available for provider review.

  • Triaging patient calls and appointment requestsusing strong nursing judgment and patient assessment skills.

  • Coordinating multidisciplinary care plansfor patients who may need involvementfromcardiology, physical therapy, occupational therapy, behavioral health, nutrition, genetics, ophthalmology, gastroenterology, and other specialty services.

  • Supporting clinic preparationby helping providers and ancillary teams stay informed, organized, and ready for complex visits.

  • Responding to patient and family questionswith clarity, compassion, and strong family-centered communication.

  • Facilitating scheduling-related coordinationand supporting provider workflows in a high-demand specialty clinic.

  • Helping build future workflows and processesas the Ehlers-Danlos Center continues to grow.

This is an excellent fit for an RN who enjoysbuildingnew programs, thrives in a fast-growing clinical environment, and is comfortable partnering across multiple specialties. Pediatric nursing experience is strongly preferred, especially experience working with pediatric patients and families.Ambulatory nursing experience in genetics, rheumatology,neurologyor complex care would be valuable to this role. Additionally, care coordination and Epic familiarityispreferred.

MINIMUM REQUIREMENTS:

Education: Bachelor of Science in Nursing from an accredited nursing program.

Experience: 1 year of registered nursing experience required, 3 years of related and relevant registered nursing experience strongly preferred.

Specialty Board Certification strongly preferred

License/Certification: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification is required.

PHYSICAL DEMANDS

Job requires standing for prolonged periods, frequently traveling, and bending/stooping. Proficient communicative, auditory, and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids, and infectious diseases.

The pay range for this role is $85,820.80 - $113,401.60 annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
