Deliver comprehensive primary care in the Bronx, managing acute/chronic conditions for diverse families in a community health setting.
Key Responsibilities:
- Conduct physicals, vaccinations, sick visits, and wellness screenings
- Manage diabetes, hypertension, asthma, and preventive care
- Order labs/imaging, prescribe medications, and coordinate referrals
- Provide patient education and chronic disease counseling
Qualifications:
- FNP certification with active NY NP license
- Primary care experience preferred; bilingual Spanish advantageous
- BLS; EHR proficiency required
