Nursing Home Nurse Practitioner - NYC Boroughs
Location: Manhattan base (10018), serving nursing homes across NYC Boroughs
Position: Nurse Practitioner - Family Practice
Provide specialized primary care to nursing home residents across NYC boroughs (Manhattan, Bronx, Brooklyn, Queens, Staten Island) from a Manhattan hub.
Key Responsibilities:
- Conduct assessments, manage chronic conditions, and med reconciliation in SNFs
- Order labs/imaging, treat infections/wounds, and coordinate hospice/palliative care
- Collaborate with facility staff, MDs, and families for care plans
- Complete regulatory documentation and quality metrics
Qualifications:
- FNP or AGNP certification with active NY NP license
- Geriatrics/long-term care experience preferred; new grads welcome
- Reliable transportation; BLS; Spanish bilingual a plus
