What are the responsibilities and job description for the Direct Primary Care Physician – Internal Medicine or Family Medicine position at The Pilot Clinic?
The Pilot Clinic is an independent, physician-owned medical practice in Goodyear, Arizona, seeking a board-certified or board-eligible Internal Medicine or Family Medicine physician to join our growing Direct Primary Care practice. We welcome both experienced outpatient physicians and newly graduating residents who are interested in building a long-term, relationship-focused primary care practice.
This position is designed for a physician who enjoys primary care but wants a more sustainable way to practice medicine: a smaller patient panel, longer visits, greater clinical autonomy, minimal insurance-related administrative burden, and the ability to build meaningful long-term relationships with patients.
Physicians are not compensated based on RVUs, coding, or high-volume visit production. Instead, compensation grows with the physician’s active Direct Primary Care panel, with a mature panel capped at approximately 600 members.
Responsibilities
- Provide comprehensive outpatient primary care to an assigned Direct Primary Care panel
- Manage preventive care, acute medical concerns, and chronic medical conditions
- Build strong long-term relationships with patients
- Provide direct patient communication through visits, phone, and secure messaging
- Participate in shared after-hours physician coverage
- Collaborate with clinical and administrative staff
- Practice evidence-based medicine
- Perform in office-based procedures according to training and interest
Practice Model
Our Direct Primary Care model emphasizes:
- Maximum panel of approximately 600 patients
- Longer, less-rushed appointments
- Same- or next-day access when clinically appropriate
- Direct physician-patient communication
- Minimal insurance-related administrative burden
- Established staff, office, EHR, equipment, billing, and membership infrastructure
- Flexible physician scheduling
- Focus on preventive medicine, cardiometabolic health, chronic disease management, and comprehensive outpatient care
Depending on experience and interest, physicians may also perform office-based procedures such as joint injections, dermatologic procedures, point-of-care ultrasound, IV fluids, and other minor procedures. Physicians are not expected to perform procedures outside their training or comfort level.
Qualifications
Required
- MD or DO degree from an accredited medical school.
- Successful completion of an ACGME-accredited U.S. residency in Internal Medicine or Family Medicine.
- Board-certified or board-eligible in Internal Medicine or Family Medicine.
- Active, unrestricted Arizona medical license or eligibility for Arizona licensure.
- Interest in providing comprehensive outpatient primary care within a Direct Primary Care model.
Preferred:
- Several years of outpatient primary care experience
- Comfort managing a broad range of adult primary care conditions
- Interest in building a long-term patient panel
- Office procedure experience is helpful but not required
We are particularly interested in physicians who have experienced traditional primary care and are looking for a more sustainable, relationship-focused model of medicine.
Compensation
W-2 compensation is tied to the physician’s active Direct Primary Care panel and increases as the panel grows.
Annual salary ranges from $90,000 to $300,000 based on active DPC panel size, with the maximum salary tier reached at a mature panel of 551–600 active members.
During the initial panel-building stage of 0–50 active members, compensation is $100 per hour for scheduled clinical and approved administrative time. Scheduled hours during this phase are based on panel size, patient-care demand, and practice needs. As the patient panel grows, scheduled clinical time will increase accordingly.
Once the panel exceeds 50 active members, compensation transitions to the panel-based salary structure and increases progressively as the physician’s panel grows.
Panel size is based on active DPC memberships and averaged over time to reduce compensation changes from short-term fluctuations.
Benefits
- W-2 employment
- 401(k) with 4% employer match
- Paid time off
- Sick leave
- Paid federal holidays observed by the practice
- $1,500 annual CME/professional development allowance
- Malpractice insurance
- Flexible scheduling
- Clinical and administrative staff support
- EHR and practice technology
- Exam rooms, medical equipment, and practice infrastructure
- Membership administration and billing support
- Shared after-hours physician coverage
