Setting: 100% Onsite

Schedule: Friday - Tuesday, 8:00am - 4:30pm

Status: Full-time/Non-Exempt/Non-Management

Job Description Summary:

With the support of and in partnership with the E6 team, the Lead Substance Use Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals in their recovery from substances and in regaining control over their lives. These teams are part of a new large-scale street-based engagement effort in the Metro Los Angeles Area (Service Area 4). This is a LA County approved strategy to combat homelessness. The goal of E6 is to create a countywide network of multidisciplinary, integrated street-based teams to identify, engage, and connect, or re-connect homeless individuals to interim and/or permanent housing and supportive services. The Substance Use Case Manager provides information and support for program members, and assists members in gaining admission to inpatient or more intensive substance abuse treatment settings as needed. The Substance Use Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations.

Job Description Essential Duties and Responsibilities:

  • Utilize harm reduction, social, environmental, and motivational, strategies to support clients, in collaboration with licensed clinicians, other counselors, case managers, and support staff.
  • Encourage and promote an environment that is strength based to assist clients in meeting their individual goals.
  • Street outreach to identified homeless persons working with the team to engage in services and accept treatment from the program staff assist with enrollment process as needed.
  • Conduct and document individual counseling sessions with clients at regular intervals determined by the individual’s needs.
  • Coach clients individually on problems with transition from streets to home, supportive independent living skills, sobriety, and accessing needed recourses in the community.
  • Provide referrals to allied critical healthy living services for clients on caseload.
  • Document and account for time spent on client contacts and activities in client record in accordance with agency and funder standards.
  • Attend and actively participate in team meetings and program-wide meetings daily.
  • Maintain a current, thorough knowledge of ongoing developments and new techniques in the fields of substance abuse, mental health and behavioral health community resources and utilize these to provide comprehensive, wrap-around services to clients.

Job Description Qualifications:

  • High School Diploma or GED or Equivalent
  • CADC I, CADC II, CADC III, or similar credentials.
  • An eagerness to provide top-notch quality services to individuals experiencing homelessness.
  • An understanding of and familiarity with homeless services and affordable housing in LA county
  • Experience in the fields of social work, public health or other types of outreach
  • Lived experience with homelessness is valued.
  • Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record preferred
  • Must be willing to transport clients

Job Description Preferred Qualifications:

  • Bachelor's Degree
  • Training in harm reduction, “housing first” principles and street-based outreach preferred

Job Description Work Environment:

  • Field (may need to travel) and indoor office environment
  • Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
  • On occasion walk or drive to different local sites.
  • Will necessitate working in busy and loud environments.
  • Will be exposed to elements like cold, heat, dust, noise and odor.
  • May need to bend, stoop, twist, and sit throughout the day.

Outreach positions might involve various types of lifting, such as carrying supplies, moving materials, or handling tools. The weight of these items can range from light to moderate, depending on the specific tasks.

Examples of Weights:

Supplies: Boxes of brochures, pamphlets, or educational materials (i.e., backpacks) could weigh between 15 -20 and pounds each.

Materials: Small items like equipment, tools, or supplies could weigh between 10 and 20 pounds.

General Lifting: It's crucial to consider the weight of items that may be moved or lifted during outreach activities.

Physical requirements like walk a distance, or navigate stairs, all of which are necessary to perform outreach work.
