Location: C3 Skid Row, 627 S. San Julian Street, Los Angeles, CA 90014

Status: Full Time, Non- Exempt (hourly) Non-Management

Summary

With the support of and in partnership with the C3 team, the Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals and is responsible for identifying, engaging, and providing support to vulnerable homeless program participants, including providing aftercare and follow up support, once housed. The C3 Case Manager will engage homeless individuals living on the street in Skid Row, to provide linkages to housing and supportive services. These teams are part of a large-scale street-based engagement effort in the Metro Los Angeles Area (Service Area 4). The goal of these teams is to create a countywide network of multidisciplinary, integrated street-based teams to identify, engage, and connect, or re-connect homeless individuals to interim and/or permanent housing and supportive services. The Case Manager provides information and support for program members and assists members in linking to a variety of resources and services as needed. The Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations. The C3 Case Manager will provide intensive case management services to a caseload of highly vulnerable individuals living on the streets of Skid Row.

Essential Duties & Responsibilities

  • Identify, engage, and provide support to program participants in the C3 program living on the streets in the Skid Row neighborhood, targeting those clients who are vulnerable due to medical, psychiatric, or addiction needs.
  • Work in collaboration with other team members to remove barriers to permanent housing; e.g., acquisition of identification, legal aid, benefits assistance, life skills acquisition
  • Work collaboratively as a team to identify appropriate services, taking into account each individual’s physical and cognitive needs.
  • Provide case management and aftercare/follow-up services to program participants placed in permanent housing
  • Report all statistics and updates for reporting purposes
  • Provide initial contact, needs assessment, information, education, and referrals to the homeless
  • Provide case management, basic services, and advocacy for program participants
  • Refer to target program participants to appropriate services (e.g., health, mental health, interim housing, etc.), and collaborate on care.
  • Develop effective, trusting relationships with clients that facilitate the provision of services
  • Work in collaboration with C3 team members and outside agency staff to connect clients to appropriate services
  • Maintain and record accurate electronic records, program participants files, and statistical information
  • Attend all case meetings, staff meetings, training, and retreats
  • Participate in CES outreach meetings

Qualifications

  • BA degree or equivalent experience
  • Demonstrated knowledge of and experience with the issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence)
  • Willingness to work in a high tolerance Harm Reduction model with clients who have multiple barriers.
  • Working knowledge of social services agencies in Metro LA (SPA 4) and LA County agencies serving the homeless
  • Excellent human relations/communication skills; ability to act professionally regardless of circumstances
  • Skilled in non-violent crisis intervention
  • Ability to self-motivate and organize time effectively and efficiently
  • Computer literate; able to use computerized database system for information management.
  • Passion for working with the population served
  • Ability to work as part of an interdisciplinary team

Work Environment:

  • Both office and field based environment working with high psychiatric acuity street based individuals.
  • Office environment with occasional field activities with Wellness participants and staff
  • Regularly required to sit, stand, bend and occasionally lift or carry up to 35 pounds
  • May be exposed to elements like cold, heat, dust, noise and odor
  • May need to bend, stoop, twist, and sit throughout the day
