The Pavilion at Crescent Lake is currently hiring an experienced MDS Coordinator to join our team!
Responsibilities of an MDS Coordinator:
Conduct patient assessments to determine care needs.
Oversee the Minimum Data Set (MDS) process and ensure timely completion.
Collaborate with the healthcare team to develop and update individualized care plans.
Ensure compliance with state and federal regulations.
Maintain accurate patient records and care plans.
Participate in quality improvement initiatives and audits.
Track and report quality measures, resident assessments, and RUG scores.
Provide guidance and education to staff members on MDS and documentation requirements.
Other duties as assigned.
Qualifications of an MDS Coordinator:
Current RN license in FL.
Proven experience as an MDS Coordinator in long-term care.
Detail-oriented with a focus on maintaining accurate records.
Ability to collaborate effectively within an interdisciplinary team.
