MDS Coordinator (RN or LPN – SNF Experience Required)

We are seeking a skilled and compassionate MDS Coordinator (RN or LPN) to join our dedicated team! Key Responsibilities:
Monitor and track Medicare residents to determine continued eligibility and appropriate benefit periods by assessing skilled levels of care.
Conduct concurrent MDS reviews to ensure accurate clinical documentation and appropriate RUGs category assignments.
Identify reimbursement opportunities through precise and compliant MDS submissions.
Use PCC-generated formats to communicate the MDS assessment schedule to the interdisciplinary team and facilitate timely care planning.
Oversee daily patient care operations, providing direction and support to nursing staff.
Coordinate care plans in accordance with regulatory requirements and clinical standards.
Ensure delivery of high-quality patient care and allocate necessary resources to support care goals.
Manage scheduling and documentation for all Medicare and Medicaid assessments.
Initiate Medicare coverage for qualified residents or issue denial letters when needed.
Stay up to date on changes in Medicare coverage policies and ensure proper documentation for reimbursement.
MDS Coordinator Qualifications:
Active RN or LPN license in the State of Florida (required)
Minimum of 2 years clinical experience in a long-term care or skilled nursing facility (required)
Previous experience as an MDS Coordinator (preferred)
Working knowledge of MDS 3.0, Medicare/Medicaid reimbursement, and case management
Strong organizational, leadership, and communication skills
