What are the responsibilities and job description for the LPN Wound Care / Restorative Nurse-Skilled Nursing Facility position at The Orchard?
- Complete ordered wound treatments and dressing changes.
- Monitor residents for new or worsening skin concerns.
- Complete wound measurements and documentation as assigned.
- Ensure treatments are completed and properly documented on TAR.
- Report changes in wounds or skin condition to the DON/RN and provider as appropriate.
- Assist with weekly skin/body audits.
- Assist residents with established restorative nursing programs.
- Work with nursing, CNAs, and therapy to carry out restorative recommendations.
- Perform duties according to policy and professionally.
- attend staff meetings and in-service programs
- Perform other assigned duties
