Senior Community Population Health Coordinator will oversee the development and execution of programming and services centered on prevention and management of chronic disease, and addressing social drivers of health. The responsibilities include identifying community health needs, developing health programs, coordinating volunteers, and working with healthcare organizations, government, community-based organizations and industry partners to improve health outcomes. Additionally, this role will analyze data, interpret reporting dashboards, and develop targeted interventions, and scalable programs across healthcare settings. Contributes to long-term staff sustainability. Collaborative efforts extend to community partners, PACT, Wexner Medical Center, and the University. Provides leadership and supervisory experience to community health workers, youth coordinator and students.

The Senior Population Health Coordinator leads population‑level initiatives to improve health outcomes for individuals and families served by the Healthy Community Center in Columbus, Ohio. The Healthy Community Center (HCC) position works collaboratively with internal OSU teams, community partners, and leadership to implement data-driven strategies and develop programming that addresses health needs across defined populations, with emphasis on building healthier communities.

The Senior Population Health Coordinator supports the Director of the OSU Healthy Community Center in establishing clinical-community linkages that connect community patients with preventive health services while creating experiential learning opportunities for OSU students. This position will create structured pathways that enable patients to receive evidence-based lifestyle interventions, while simultaneously providing OSU students with hands-on training in community health settings.

The Senior Population Health Coordinator is also responsible for population health data collection and analysis, developing and maintaining dashboards to track patient outcomes and program metrics, and conducting comprehensive program evaluations to assess effectiveness and inform continuous quality improvement. Additionally, they will supervise assigned staff and represent the Healthy Community Center on select committees, coalitions, and community meetings related to population health and health equity.

Key responsibilities include health and wellness program data management, collaborating with clinical partners to streamline referral processes, building cross sector partnerships, ensuring quality service delivery, and using data-driven insights to optimize programs and demonstrate impact.

The ideal candidate will have experience in community health programming, healthcare partnership development, demonstrated data analysis and visualization, and a strong understanding of chronic disease prevention and management through lifestyle modification.

Minimum Required Qualifications

Master's Degree preferred. Bachelor's degree required. Advanced degree in relevant field may be desirable. 4 years of relevant experience required. 4-8 years of relevant experience preferred.
