In addition to providing specialized short-term rehabilitation in partnership with leading hospitals and person-directed nursing home care, The New Jewish Home offers programs to help older adults in the community thrive. We operate comprehensive adult day health care programs in Manhattan and the Bronx, a home care agency, a certified home health agency, and a Medicaid Assisted Living Program. The Manhattan Division of The New Jewish Home is searching for a full time MDS Coordinator RN to join their team. This position sits in our 106th , NY nursing home location. The RAUM Manage will: Assume MDS responsibility for a caseload to of post-acute patients and long term residents; scheduling; completing and coordinating completion of the MDS and RAI process Issue Notices of Medicare Non Coverage and Detailed Notices and SNF ABNs Determine if patients meet Medicare coverage eligibility on admission and monitor for ongoing coverage during patient/resident stays Work with IDT to insure best practice & compliance in RAI process RAC-CT AANAC accreditation preferred 3-5 years recent experience as an MDS Coordinator at a minimum Proficiency in current MDS coding rules and Medicare coverage regulations Valid License and currently registered as a professional RN in NYS Salary Range: $92, 267 to $115,000 per annum The New Jewish Home (TNJH) provides a salary range in compliance with New York City's Salary Transparency in Job Advertisements Law.
