Overview
We are seeking a dedicated and experienced Wellness Nurse to join our healthcare team. The ideal candidate will possess a strong background in patient care across diverse medical settings, including acute, long-term, and community-based care. The Wellness Nurse will play a vital role in providing comprehensive nursing services, supporting patient health and wellness, and ensuring adherence to healthcare standards such as HIPAA and NCQA. Experience with electronic health records PCC (EHR) is highly desirable. This position offers an opportunity to work in a dynamic environment focused on holistic patient care, including geriatrics, behavioral health, pediatrics, and specialized medical treatments.
Responsibilities (Including but not limited to- More details upon hire)
- Conduct thorough patient assessments including vital signs, physical examinations, and health histories using advanced clinical skills.
- Administer medications and injections safely following aseptic techniques and medication protocols.
- Manage complex care plans involving IV infusions, catheterizations, tube feeding, ostomy care, wound management, and ventilator support.
- Coordinate diagnostic evaluations such as laboratory tests, phlebotomy, sonography, mammography, and other imaging procedures.
- Provide specialized care for diverse populations including geriatrics, pediatrics, neonatal patients, individuals with developmental disabilities or dementia care needs.
- Support acute pain management, trauma medicine protocols (Level I & II Trauma Centers), infectious disease control, hospice & palliative medicine practices.
- Document patient care accurately within EMR/EHR systems ensuring compliance with HIPAA and other regulatory standards.
- Participate in discharge planning, case management, health coaching, and patient education initiatives.
- Collaborate with multidisciplinary teams across outpatient clinics, inpatient units (ICU, PICU), emergency rooms (ER), and post-acute settings such as skilled nursing or assisted living facilities.
- Maintain sterile processing standards for procedures like suturing or ostomy care when applicable.
- Stay current with medical terminology updates including ICD coding (ICD-9/10), CPT coding for procedures.
Qualifications
- Valid LPN/RN license with current state licensure required; additional certifications such as BLS/CPR preferred.
- Extensive experience in diverse healthcare environments preferred
- Proficiency with EMR/EHR systems
- Experience working with vulnerable populations such as children (including neonatal), elderly patients in memory or long-term care facilities or individuals with disabilities.
- Ability to work independently while collaborating effectively within multidisciplinary teams in fast-paced environments such as trauma centers or urgent care clinics.
- Excellent communication skills with a compassionate approach to patient-centered care; familiarity with medical documentation standards is essential.
