Job Type: Full-time (40 hours/week) Monday-Friday schedule

Program Description: 

The ProHealth Integrated Care team brings together behavioral health, physical health, wellness, care coordination, and occupational therapy supports helping clients build healthier, more functional lives while reducing barriers to care. Within this integrated model, occupational therapy helps children and adolescents strengthen the skills, routines, regulation strategies, and daily participation needed to thrive at home, in school, and in the community. 


Position Summary: 

Join MHCGM’s ProHealth Integrated Health Program as an Occupational Therapist and help expand innovative, community-based occupational therapy services for children and adolescents. As part of a Certified Community Behavioral Health Clinic, the Occupational Therapist role supports youth and families through an integrated model that connects behavioral health, physical health, and functional skill development. 

The Child and Adolescent Occupational Therapist will work with youth, caregivers, schools, community partners, and multidisciplinary providers to address occupational performance needs that affect daily routines, emotional regulation, sensory processing, executive functioning, social participation, school engagement, self-care, and overall wellness. The Occupational Therapist plays a key role in developing best practices for pediatric and adolescent occupational therapy within an integrated health setting.   

Requirements

 

Description of Duties 

  • Provide child- and adolescent-focused occupational therapy services in home, school, and community settings to address occupational performance and participation needs. 
  • Report to the Director of ProHealth Integrated Care. 
  • Complete occupational therapy evaluations, develop individualized treatment plans, and monitor patient progress toward functional goals. 
  • Deliver interventions that support emotional regulation, sensory processing, daily routines, self-care, executive functioning, school participation, social engagement, and family-identified priorities. 
  • Provide caregiver education, coaching, and family support services to strengthen skills across natural environments. 
  • Collaborate with ProHealth, behavioral health, primary care, school-based, and community providers to coordinate integrated care. 
  • Communicate and collaborate with the Integrated Occupational Therapy Team to support consistent, high-quality service delivery. 
  • Participate in team meetings, case consultations, and program development activities as determined by supervisor. 
  • Practice in a manner that reflects MHCGM’s values, mission, and commitment to trauma-informed, person- and family-centered care. 
  • Flexibly support occupational therapy evaluations or services for individuals across the lifespan when program needs arise. 
  • Provide clinical supervision to Occupational Therapy Assistants as needed. 
  • Document all services in accordance with agency policies and applicable state and federal standards. 
  • Maintain required professional licenses, certifications, memberships, and accreditations. 
  • Perform all other duties as assigned. 
  • Transport clients, as appropriate and in accordance with agency policies, to community-based activities and settings where they can participate in meaningful occupations and practice targeted functional skills.

Education: 

  • Master’s or Doctorate Degree OTR/L preferred
  • Active New Hampshire state OT license

Qualifications and Skills 

  • Experience working with children, adolescents, and families strongly preferred.
  • Knowledge of pediatric and adolescent occupational therapy interventions related to sensory processing, emotional regulation, executive functioning, daily living skills, school participation, and social engagement.
  • Ability to exercise sound clinical judgment and apply evidence-informed occupational therapy practices in community-based and integrated health settings.
  • Flexibility and adaptability to respond to varying environments, family needs, developmental stages, diagnoses, and presentations.
  • Self-directed while remaining collaborative and team oriented.
  • Strong communication, documentation, organization, and caregiver coaching skills.
  • Reliable transportation, clean driving record, and proof of auto insurance required.
