Role Description The Hospice Patient Care Manager is a full-time, on-site role based in Livonia, MI. This position is responsible for overseeing and coordinating hospice patient care services, ensuring that each patient receives compassionate, high-quality care aligned with clinical standards and regulatory requirements. The Hospice Patient Care Manager leads and supports interdisciplinary care teams, develops and updates patient care plans, and monitors outcomes to maintain consistency and safety in care delivery. Daily responsibilities include supervising nursing staff, providing guidance and training, collaborating with physicians and other healthcare professionals, and communicating with patients and families to address needs and preferences. The role also involves documentation review, compliance with hospice policies and procedures, and participation in quality improvement initiatives.

Qualifications
- Demonstrated ability to provide and oversee Patient Care, Quality Patient Care, and Direct Patient Care in a hospice or home care setting.
- Experience with Training and mentoring clinical staff to support best practices and continuous improvement.
- Strong Nursing skills, with the ability to coordinate interdisciplinary care and support complex patient needs.
- Current Registered Nurse (RN) license in the state of Michigan, with hospice or home health experience preferred.
- Knowledge of Medicare, Medicaid, and other payer requirements related to hospice care.
- Excellent communication, interpersonal, and organizational skills, with the ability to work collaboratively with diverse teams and families.
- Proficiency in clinical documentation and use of electronic health record systems.
- Ability to prioritize multiple responsibilities, maintain confidentiality, and uphold professional and ethical standards.
