What are the responsibilities and job description for the Behavioral Health Nurse position at The Medical Team, Inc?
Essential Functions:
- Performs case assessments and participates in the development and maintenance of treatment plans in accordance with The Journey Program.
- Develops, implements, and communicates patient plans of care.
- Completes nursing assessment forms in a timely manner.
- Conducts comprehensive biopsychosocial - spiritual assessments.
- Medication monitoring and administration.
- Collaborates with internal and external providers to coordinate psychiatric treatment with medical treatment.
- Participates in case conferences.
- Provides information regarding patients rights, staff assignments, and plans of care to patient and/or family members.
- Involves patients and family members in developing and implementing the plan of care whenever possible.
- Implement current nursing practice according to the Psychiatric Mental Health Nursing Standards (ANA).
- Assesses patient condition on admission and weekly thereafter as long as the patient remains in Agency care.
- Monitors clinical interventions for compliance with the defined plan of care.
- Analysis assessment data in determining a diagnosis.
- Develop a plan of care with input of the patient, other nurses, family and health care team.
- Identifies expected outcomes individualized to the patient.
- Implements and structures skilled nursing interventions to meet patient needs and changed conditions as identified by the plan of care.
- Provides, structures and maintains a therapeutic environment in collaboration with the patient and other health care providers.
- Employs strategies and interventions to promote and maintain mental health and prevent mental illness.
- Responsible for acting as a case manager and communicating across disciplines to coordinate the patient's care
- For timely, accurate, and comprehensive documentation using the documentation required by The Medical Team for the Journey Behavioral Health Home Care Program.
- Communicates goals and service offerings to the community and provide public service/community educational programs.
- Responsible for acting in a caring, professional, and ethical manner for participating in the scheduled clinical meetings.
- Participates in professional growth and development.
- Participates in regular educational programs.
- Demonstrates current knowledge of principle of basic nursing and psychiatric nursing practices.
- Demonstrates current knowledge of a coordinated interdisciplinary approach the provision of home health services.
- Participates in the development and presentation of educational programs as appropriate.
- Maintains current professional license.
- Attends Agency inservices.
- Participates in the Quality Assessment and Improvement program as required.
- Fulfills additional performance responsibilities as assigned and necessary.
