What are the responsibilities and job description for the Case Manager position at The Martinsburg Initiative?
Job Overview
The Martinsburg Initiative (TMI) is hiring a Police Support Specialist focused on courts, reentry, and community stabilization.
In this role, you will help individuals navigate important transitions—from incarceration back into the community, from a sobering-center stay into ongoing support, or from a court referral into treatment and services. You will work with participants to identify their priorities, develop practical plans, and address barriers involving housing, healthcare, recovery, identification, benefits, and family needs.
This position combines direct case management with partnerships across courts, corrections, law enforcement, and community organizations. It is a good fit for someone who builds trust, follows through, and wants to help people establish greater stability and reduce repeated crisis and justice-system involvement.
What You’ll Do
- Coordinate court and reentry referrals. Work with municipal and treatment courts, jails, prisons, and community partners to establish clear referral and follow-up arrangements.
- Support transitions from incarceration. Help participants prepare for release and connect with services afterward, including treatment, recovery support, housing, healthcare, benefits, identification, and transportation.
- Provide sobering-center case management. Engage participants, identify needs, coordinate service connections, and follow up after discharge.
- Develop individualized service plans. Work with participants to identify strengths, priorities, family needs, and achievable goals.
- Support prevention and recovery. Recognize how substance use, overdose risk, trauma, adverse childhood experiences, housing instability, and other challenges affect a person’s needs.
- Connect people with services. Arrange warm handoffs, help resolve access barriers, and confirm that participants connect with the resources they need.
- Coordinate with Community Cup. Serve as a TMI liaison, working alongside peers and existing outreach staff to support referrals and ongoing engagement.
- Provide timely follow-up. Complete assigned post-overdose outreach and continuing case management, coordinating with crisis staff when additional support is needed.
- Participate in coordinated community response. Complete Situation Table training and contribute to appropriate referrals and assigned follow-up.
- Maintain accurate records. Document services, referrals, plans, and outcomes in Julota and participate in supervision, case review, and program improvement.
Required Qualifications
- Bachelor’s degree in social work, psychology, counseling, human services, or a closely related field from an accredited institution.
- Relevant experience in case management, behavioral health, substance-use services, family support, community services, or public safety partnerships.
- Strong engagement, communication, organization, documentation, and follow-through skills.
- Ability to maintain professional boundaries and work respectfully with participants, families, peers, and justice-system partners.
- Ability to meet applicable licensure and supervision requirements for assigned duties.
- Valid driver’s license and ability to meet agency driving, background, confidentiality, and partner-agency requirements.
Preferred Qualifications
- Experience with reentry, corrections, municipal or treatment courts, intensive case management, or substance-use prevention and recovery.
- Current professional licensure or eligibility for licensure in a relevant field.
- Master’s degree in a related field.
Schedule and Work Environment
This position reports to TMI’s Assistant Director. Work takes place at police departments, courts, correctional settings, the sobering center, homes, and other community locations. Local travel is required. The full-time schedule will reflect participant and program needs, including scheduled evening or weekend assignments when needed.
You will coordinate with other TMI staff so each assigned participant has a clear service plan, an identified lead worker, and consistent follow-up.
Help people build a practical path forward through reentry, recovery, and connection to their community.
