Position Summary

The Clinical Services Nurse serves as The Listen Center's primary nursing resource, providing clinical oversight, healthcare coordination, consultation, and quality assurance support for individuals receiving services. The position focuses primarily on Residential Services while providing consultation and support to Day Services as needed.

The Clinical Services Nurse works collaboratively with staff, guardians, healthcare providers, pharmacies, and community partners to promote safe, person-centered services and positive health outcomes. This position emphasizes healthcare coordination, clinical oversight, regulatory compliance, and quality improvement rather than routine direct nursing care.

Primary Responsibilities

Clinical Oversight

  • Serve as the primary nursing resource for Residential Services.
  • Monitor the health status and healthcare needs of individuals receiving services.
  • Review physician orders, treatment recommendations, and health-related documentation.
  • Provide clinical guidance related to medications, chronic health conditions, wellness, and healthcare supports.
  • Conduct nursing assessments and evaluations as needed.

Healthcare Coordination

  • Coordinate communication among residential staff, guardians, healthcare providers, pharmacies, hospitals, and specialists.
  • Support continuity of care following hospitalizations, emergency room visits, and significant changes in health status.
  • Ensure physician orders and treatment recommendations are communicated and implemented appropriately.
  • Assist with healthcare planning and follow-up as needed.

Medication System Oversight

  • Maintain oversight of Medication Administration Records (MARs), including MAR management for individuals attending Day Services.
  • Review medication-related incidents and identify trends, concerns, and opportunities for improvement.
  • Provide guidance regarding medication-related policies, procedures, and documentation requirements.

Compliance and Quality Assurance

  • Maintain nursing documentation in accordance with applicable regulations, licensing standards, and organizational policies.
  • Assist with investigations, corrective actions, and follow-up related to health and safety concerns.
  • Support licensing reviews, accreditation activities, and quality improvement initiatives.
  • Assist with the development and revision of health-related policies and procedures.

Consultation and Collaboration

  • Serve as a clinical consultant to staff and leadership.
  • Support staff in understanding and implementing healthcare recommendations and support plans.
  • Collaborate with organizational healthcare partners to address the health needs of individuals receiving services.
  • Promote health, safety, and person-centered practices throughout the organization.

Qualifications

  • Current Registered Nurse (RN) license in North Dakota.
  • Current CPR certification (or ability to obtain upon hire).
  • Minimum of three years of nursing experience preferred.
  • Experience working with individuals with intellectual and developmental disabilities (I/DD) preferred.
  • Knowledge of person-centered practices, medication administration, and health care coordination.
  • Strong assessment, organizational, and problem-solving skills.
  • Excellent written and verbal communication skills.
  • Proficient in electronic health records and Microsoft Office; experience with Therap is preferred.
  • Ability to work independently while collaborating effectively with interdisciplinary teams.
  • Valid driver's license and ability to travel between program locations as needed.
