RESPONSIBILITIES:

The Coordinator of the Case Management/Recovery Support Program (CM/RSP) is generally responsible for managing the day-to-day operation of the CM/RSP. The CM/RSP provides community-based solutions for individuals who are at risk of psychiatric hospitalization and who are poorly engaged in clinical care. The primary goal of the CM/RSP is to ensure that TLOC clients remain stable within the community and avoid the unnecessary use of psychiatric emergency and inpatient facilities. The CM/RSP Coordinator is responsible for ensuring that TLOC clients are firmly engaged in community-based care and that their holistic needs are being addressed.

Specific duties include:

· Collaborate with other departments within TLOC to ensure continuity of care for clients assigned to the CM/RSP.

· Monitors Case Assignments for staff and ensures that clients assigned to the CM/RSP are seen no less than once monthly for wellness checks, medication monitoring, community support, etc.

· Routinely collaborates with the Associate Director of Clinical Operations, Care Coordinator and the EHR Systems Administrator as needed to develop and implement data metrics for monitoring the frequency (SDOH) and type of services provided to clients.

· Conduct staff training as indicated by trends in reported data.

· Provides training and support to staff to ensure adherence to evidence-based practices and fidelity to established service model(s).

· Provides weekly supervision to CM/RSP staff.

· Review and sign-off on documentation as needed.

· Collaborate with The Living Room Program staff and CST staff and other program staff in need of support as requested.

· Conduct Outreach and Engagement activities to include attending community events and canvassing as needed.

· Monitors and ensures that documentation of Case Management Services (encounter notes) are completed in a timely manner and in accordance with established policies and procedures.

· Ensures that CM/RSP program productivity standards are consistently met.

· Maintains an active caseload of 10 clients assigned by the CCBHC Program Director or the Associate Director of Clinical Operations.

· Participates in the Quality Assurance and Utilization review process as assigned by the Director of Clinical Operations.

· Participate in IDT meetings as scheduled.

· Assign CM/RSP caseload within 24-48 hours of referral and monitors CM/RSP caseloads weekly.

· Ensure CM/RSP completes an SDOH Needs Assessment and Service Plan during initial meeting with assigned client.

· Ensure CM/RSP Team Members complete a Person-Centered Wellness Recovery Action Plan within 7 business days of initial contact.

· Coordinate with clinicians to update client’s MDTP under “Client Goals” section to reflect identified CM Goals.

· Other duties as assigned.

qualifications and education requirements:

· Must meet criteria for qualification as a QMHP under IL. Administrative Code Rule 132.

· Demonstrated knowledge of JCAHO accreditation standards.

· Demonstrated knowledge of IL Administrative Rules 132, 140, and 2060.

· High degree of professionalism in dealing with diverse groups of people, including clients, staff, and others.

· Demonstrated ability to work collaboratively with other departments within TLOC.

· Ability to make appropriate, informed decisions.

· Ability to multitask, complete a high volume of tasks and projects with little or no guidance.

· Well-developed organizational and time management skills.

· Effective verbal and written communication skills.

· Professional discretion and adherence to confidentiality.

· Must be culturally competent, empathetic, and able to interact and develop rapport with a diversified population.

· Must have 3-5 years supervisory experience.

· Must have an ability to encourage, lead and motivate team members.

· Must have moderate to advanced computer skills.

Pay: $80,000.00 per year
