Duties
- Conduct assessments to understand each client’s needs and eligibility for services.
- Develop and implement personalized case plans that match clients’ goals and realities.
- Offer mentorship and steady support to foster growth and independence.
- Use motivational interviewing to help clients embrace positive change.
- Coordinate with community resources — from housing and Medicare to mental health and addiction support programs.
- Track progress and adjust plans to ensure clients stay on course.
- Keep detailed, accurate records of all sessions, updates, and referrals.
- Advocate fiercely for client rights and access to essential services.
- Guide and supervise interns or junior staff when needed.
Requirements
- Bachelor’s degree in Social Work, Psychology, or a related field (Master’s preferred).
- Experience in behavioral health, mental health, or child welfare settings is a major plus.
- Strong grasp of workforce development and case management principles.
- Skilled in motivational interviewing and communication.
- Highly organized and self-motivated with sharp attention to detail.
- Works well independently and thrives within a multidisciplinary team.
- Supervisory or mentoring experience is an advantage.
- Knowledge of addiction and recovery support methods is helpful.
