Registered Nurse

Position Overview

We are seeking a compassionate and detail-oriented Registered Nurse (RN) – Home Health to provide skilled nursing care to patients in their homes. This role involves managing patient caseloads, delivering personalized care, and collaborating with interdisciplinary teams to ensure optimal patient outcomes.

Work Schedule

• Schedule: Monday – Friday (may vary based on patient needs)
• Hours: Day shift (flexible based on visits)
• On-Call/Weekend Requirement: As needed
  
  

Key Responsibilities

• Conduct initial patient assessments and admissions
• Develop and implement individualized care plans
• Monitor patient condition and track progress toward goals
• Administer medications and treatments as prescribed
• Provide wound care, IV therapy, and disease management as applicable
• Educate patients and caregivers on care plans and health management
• Coordinate care with physicians, therapists, and other healthcare providers
• Maintain accurate and timely clinical documentation
• Ensure compliance with home health regulations and standards
  
  

Required Qualifications

• Active Registered Nurse (RN) License
• Experience in home health or clinical nursing
• Valid driver’s license and reliable transportation
