Registered Nurse

Key Responsibilities

• Perform Start of Care (SOC) visits
• Complete OASIS documentation accurately
• Conduct routine skilled nursing visits
• Perform recertification and discharge visits
• Medication reconciliation
• Patient education and preventive care
• Coordinate home health services and referrals
• Collaborate with providers and care team
• Same-day documentation
• Manage assigned caseload and perform patient assessments
• Deliver wound care, catheter care, and other skilled nursing services
• Educate patients and caregivers on disease processes and care plans
• Provide medication education and management
• Document care accurately using EMR systems
• Collaborate with interdisciplinary care teams to ensure continuity of care
• Meet productivity expectations and quality standards
  
  

Productivity Expectations: Expected Productivity: 30-40 points per week

Required Qualifications

• Active Registered Nurse (RN) license
• Home health nursing experience (required)
• Ability to work independently in a field-based setting
• Strong clinical, assessment, and patient education skills
• Willingness to participate in on-call rotations
• Must pass required occupational health screenings
