Travel RN – Home Health / Hospice

Location: East Baton Rouge & Livingston Parish Schedule: Monday–Friday, 8:00 AM–4:30 PM Weekly Hours Guaranteed: 32 On‑Call / Weekend Rotation: Yes

Position Overview

We are seeking an experienced Travel Registered Nurse to join our Home Health/Hospice team. This role provides compassionate, high‑quality care to patients in their homes while meeting productivity and documentation expectations. The ideal candidate is self‑directed, organized, and comfortable managing a field‑based caseload.

Key Responsibilities

• Deliver skilled nursing care to home health and hospice patients across East Baton Rouge and Livingston Parish.
• Complete comprehensive assessments, develop care plans, and provide ongoing patient education.
• Document all visits and interventions accurately within HCHB (Homecare Homebase).
• Collaborate with interdisciplinary team members to ensure coordinated, patient‑centered care.
• Meet productivity expectations of 30 points per week.
• Participate in required on‑call and weekend rotations.
• Travel between patient homes; mileage reimbursed from first to last patient.
  
  

Required Qualifications

• Active RN license (state‑appropriate).
• Minimum of one year of Home Health experience.
• Proficiency with HCHB (Homecare Homebase).
• Strong clinical assessment and critical‑thinking skills.
• Valid driver’s license and reliable transportation.
  
  

Preferred Skills (Nice To Have)

• Previous hospice experience.
• Strong communication and time‑management abilities.
• Ability to work independently in a fast‑paced field environment.
  
  

Additional Details

• Dress Code: Royal blue, black, or navy scrubs.
• Mileage: Reimbursed between visits (first patient to last).
