Job Title: Nurse Practitioner
Location: Coventry/Kent County area, RI
Duration: 6-8 months possible Contract extensions
Shift [08:00 AM to 05:00 PM] Monday to Friday
Hours per week: 40 hours per week
Role Overview:
As part of an interdisciplinary team, you will provide longitudinal, integrated care for high-risk members within four skilled nursing facilities in the Coventry/Kent County area. You will manage both acute and chronic conditions, focusing on improving patient outcomes through comprehensive assessments and proactive care planning.
Key Responsibilities
- Conduct comprehensive physical assessments and document conditions using ICD-10 coding.
- Manage complex acute and chronic medical/behavioral conditions.
- Lead advanced illness and care planning conversations with patients and families.
- Collaborate closely with an interdisciplinary care team to coordinate delivery of care.
Required Qualifications
- Licensure: Active, unrestricted NP or PA license in Rhode Island.
- Certification: * NP: Board Certified (AANP or ANCC) in Family, Adult, Gerontology, or Adult-Gerontology Acute Care.
- PA: Board Certified by the NCCPA.
- Prescriptive Authority: Current, active DEA licensure.
- Logistics: Access to reliable transportation to travel between 4 facilities in Kent County (Coventry, RI area).
- Compliance: Full COVID-19 vaccination and ability to pass a drug screen.
Preferred Experience
- 1 year of clinical practice (Long-Term Care/SNF setting preferred).
- Strong understanding of Geriatrics and Chronic Illness management.
