Key Responsibilities
- Conduct in-home health assessments and preventative care evaluations.
- Complete thorough and accurate clinical documentation within required timelines.
- Identify health risks, gaps in care, and opportunities for intervention.
- Maintain compliance with clinical, regulatory, and HIPAA requirements.
- Collaborate with clinical leadership and support teams as needed.
- Participate in required training sessions and meetings.
- Deliver a positive patient experience while providing high-quality care.
- Flexible scheduling with daytime and evening availability.
- Appointment times typically range from 60–75 minutes.
- Minimum commitment of 15 completed visits per month.
- Travel required within an assigned service area (up to approximately a 50-minute radius).
Education
- Master's or Doctoral degree from an accredited Nurse Practitioner program (FNP, AGNP, ANP) or
- Master's degree from an accredited Physician Assistant program.
- Primary Care (PCP) and/or Home Health experience strongly preferred.
- Experience conducting in-home assessments is highly preferred.
- Experience working with Medicare and Medicaid populations is a plus.
- Strong clinical assessment and documentation skills.
- Proficiency with electronic medical records (EMR/EHR) systems.
- Excellent verbal and written communication skills.
- Ability to work independently and manage time effectively.
- Strong critical thinking and decision-making abilities.
- Comfortable providing care within patients' homes and community settings.
- Current, unrestricted NP or PA license in the state of practice.
- National board certification appropriate to specialty.
- Current BLS certification.
- Valid driver's license and reliable transportation.
- Ability to obtain and maintain required payer credentialing, including Medicare and Medicaid participation.
- Ability to travel throughout the assigned territory.
- Ability to lift up to 20 pounds.
- Ability to bend, kneel, and perform assessments in various home environments.
