Nurse Practitioner

Position Overview

We are seeking a Per Diem Nurse Practitioner (NP) or Physician Assistant (PA) to conduct in-home health assessments for adult and senior patients. Providers will perform comprehensive preventative health evaluations, document findings, and help identify healthcare needs and opportunities for improved patient outcomes.

This is a flexible, field-based opportunity for clinicians who enjoy working independently while making a meaningful impact in patients' lives.

Key Responsibilities

• Conduct in-home health assessments and preventative care evaluations.
• Complete thorough and accurate clinical documentation within required timelines.
• Identify health risks, gaps in care, and opportunities for intervention.
• Maintain compliance with clinical, regulatory, and HIPAA requirements.
• Collaborate with clinical leadership and support teams as needed.
• Participate in required training sessions and meetings.
• Deliver a positive patient experience while providing high-quality care.
  
  

Schedule & Expectations

• Flexible scheduling with daytime and evening availability.
• Appointment times typically range from 60–75 minutes.
• Minimum commitment of 15 completed visits per month.
• Travel required within an assigned service area (up to approximately a 50-minute radius).
  
  

Qualifications

Education

• Master's or Doctoral degree from an accredited Nurse Practitioner program (FNP, AGNP, ANP) or
• Master's degree from an accredited Physician Assistant program.
  
  

Experience

• Primary Care (PCP) and/or Home Health experience strongly preferred.
• Experience conducting in-home assessments is highly preferred.
• Experience working with Medicare and Medicaid populations is a plus.
  
  

Skills

• Strong clinical assessment and documentation skills.
• Proficiency with electronic medical records (EMR/EHR) systems.
• Excellent verbal and written communication skills.
• Ability to work independently and manage time effectively.
• Strong critical thinking and decision-making abilities.
• Comfortable providing care within patients' homes and community settings.
  
  

Licensure & Certifications

• Current, unrestricted NP or PA license in the state of practice.
• National board certification appropriate to specialty.
• Current BLS certification.
• Valid driver's license and reliable transportation.
• Ability to obtain and maintain required payer credentialing, including Medicare and Medicaid participation.
  
  

Physical Requirements

• Ability to travel throughout the assigned territory.
• Ability to lift up to 20 pounds.
• Ability to bend, kneel, and perform assessments in various home environments.
