The Judge Group is currently seeking Nurse Practitioners (NPs) and/or Physician Assistants (PAs) to conduct in-home health assessments across Ontario, New York.

Position Summary
- Providers will conduct in-person, no-cost annual preventive health risk assessments for members enrolled in Medicare, Medicaid, and Affordable Care Act (ACA) plans.
- Assessments take place in the member’s home and must meet all health plans, quality and service expectations.
- Minimum requirement: 15 visits per month.
- Providers must be credentialed with Excellus, NYS Medicare, and Medicaid to avoid onboarding delays.
- Candidates must be willing to travel within a 50-minute radius.
- Flexible daytime and evening availability
- Appointment duration: 60–75 minutes
- Paid at market rate per completed assessment
- Mileage reimbursement after the first 30 miles driven per day, at the current IRS rate
- Complete required self-paced and instructor-led training within assigned timeframes
- Provide a minimum of 15 appointment slots per month
- Deliver high-quality assessments meeting member satisfaction and health plan standards
- Submit chart notes within 24 hours of each visit
- Respond to charting queries within 48 hours
- Maintain consistent communication with clinical leadership
- Attend required training sessions and meetings
- Enter member homes safely and respectfully
- Lift up to 20 pounds as needed
- Bend or kneel for extended periods during assessments when required
- Strong computer proficiency and experience with electronic medical records (EMRs)
- Understanding of CMS and ACA risk-adjustment models
- Accurate and thorough clinical documentation skills
- Effective verbal and written communication with members and clinical teams
- Ability to maintain confidentiality and comply with HIPAA regulations
- Sound decision-making within established policies and procedures
- Adaptability to changing conditions and responsibilities
- Positive, collaborative approach with members and colleagues
- Proactively anticipate and resolve member, provider, and health plan concerns
- Master’s or Doctorate degree from an accredited NP program (FNP, AGNP, ANP) or Master’s degree from an accredited PA program
- Home health experience within the last 5 years (Required)
- 1 year of in-home assessment experience (Highly Preferred)
- Current RN and NP or PA licensure in the state of practice
- Board certification through ANCC, AANP, or NCCPA
- Active Medicaid ID for the state
- Active Medicare PTAN
- Current BLS certification
- Valid, unencumbered state driver’s license
- Proof of adequate auto insurance for the state where visits occur'
