Position Overview

We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to join our growing team. In this role, you will provide high-quality, patient-centered care to individuals in their homes, helping them achieve optimal outcomes while maintaining independence.

Schedule & Work Details

Weekly Hour Guarantee: 32–40 hours

Schedule: Monday through Friday, 8:00 AM – 4:30 PM

Productivity Expectation: 30 points

Mileage Reimbursement: Yes – reimbursed between visits (from first patient to last patient)

Coverage Area

This position will provide care across the following Mississippi counties: Jones, Jasper, Wayne, Forrest, Covington, Smith, and Lamar

This Job Will Have The Following Responsibilities

• Perform comprehensive patient assessments related to the primary diagnosis
• Develop, implement, and evaluate nursing interventions to achieve goals outlined in the patient’s plan of care
• Conduct medication reconciliation at every visit, ensuring accuracy and patient understanding
• Provide skilled nursing care while monitoring patient progress and reporting changes in condition
• Educate patients and caregivers on disease management, medications, and care plans
• Collaborate with the interdisciplinary care team to ensure coordinated, high-quality care
• Complete timely and accurate clinical documentation
  
  

Qualifications & Requirements

• Active Registered Nurse (RN) license
• Home health experience required
• Homecare Homebase (HCHB) experience preferred
• Valid driver’s license and ability to travel within the assigned coverage area
