Schedule & Work Details
Weekly Hour Guarantee: 32–40 hours
Schedule: Monday through Friday, 8:00 AM – 4:30 PM
Productivity Expectation: 30 points
Mileage Reimbursement: Yes – reimbursed between visits (from first patient to last patient)
Coverage Area
This position will provide care across the following Mississippi counties: Jones, Jasper, Wayne, Forrest, Covington, Smith, and Lamar

This Job Will Have The Following Responsibilities
- Perform comprehensive patient assessments related to the primary diagnosis
- Develop, implement, and evaluate nursing interventions to achieve goals outlined in the patient’s plan of care
- Conduct medication reconciliation at every visit, ensuring accuracy and patient understanding
- Provide skilled nursing care while monitoring patient progress and reporting changes in condition
- Educate patients and caregivers on disease management, medications, and care plans
- Collaborate with the interdisciplinary care team to ensure coordinated, high-quality care
- Complete timely and accurate clinical documentation
- Active Registered Nurse (RN) license
- Home health experience required
- Homecare Homebase (HCHB) experience preferred
- Valid driver’s license and ability to travel within the assigned coverage area
