Registered Nurse (RN) – Home Health

Location: Ville Platte, LA

Service Area: 50-mile radius from 735 W. Main St., Ville Platte, LA 70586

Schedule: Monday–Friday, 8:00 AM–4:30 PM (Weekend call once per month)

Position Overview

We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide high-quality, patient-centered care in patients’ homes. This role requires daily travel to multiple patient residences and the ability to work independently while maintaining strong communication with the interdisciplinary care team.

Key Responsibilities

• Provide skilled nursing care to home health patients in their homes
• Conduct comprehensive patient assessments and ongoing evaluations
• Administer IV therapies and medications as ordered
• Perform wound care, catheter care, and catheter changes
• Obtain lab draws and accurately document results
• Educate patients and caregivers on disease processes, medications, and care plans
• Coordinate care with physicians and other healthcare professionals
• Complete timely and accurate documentation using Homecare Homebase (HCHB)
• Ensure compliance with all home health regulations, policies, and standards of care
• Travel daily between multiple patient homes within the assigned territory
  
  

Required Qualifications

• Active Registered Nurse (RN) license
• Prior home health nursing experience (required)
• HCHB (Homecare Homebase) training and experience (required)
• BCLS certification
• Valid driver’s license and reliable transportation
• Ability to work independently in a home-based setting
  
  

Preferred Qualifications

• Minimum of 1 year of home health nursing experience
• Strong organizational and time-management skills
• Excellent communication and patient-education abilities
