Position Overview:The Hospice Registered Nurse (RN) provides compassionate, patient‑centered end‑of‑life care to hospice patients in their homes and facility‑based settings. This role supports comprehensive nursing assessments, care coordination, and on‑call coverage while working closely with the interdisciplinary hospice team to ensure comfort, dignity, and quality of life for patients and families. The RN must be comfortable navigating sensitive end‑of‑life conversations and supporting patients and families through complex emotional situations.
Schedule & Work Details:- Location: Lafayette, LA, United States
- Coverage Area: 50‑mile radius from the Jonesboro branch office
- Schedule: Monday through Friday
- Shift Hours: 8:00 AM - 4:30 PM
- Weekly Hour Guarantee: 40 hours
- Hourly Rate: $53.00 - $55.00 per hour
- Productivity Expectation: At least 26 points per week
- On‑Call Requirement:
- One weekday per week and one weekend per month
- Weekend on‑call runs from Friday at 4:30 PM through Monday at 8:00 AM
- Weekday on‑call runs from 4:30 PM through 8:00 AM the following day
- On‑call duties include supporting the primary on‑call nurse with PRN and scheduled patient visits after business hours
- Mileage Reimbursement: Yes — reimbursed between patient visits from first patient to last patient of the day
- Assist with hospice patient visits and perform comprehensive nursing assessments
- Complete recertifications, patient deaths, and discharges as appropriate
- Order medications and medical supplies based on patient care needs
- Coordinate care with facilities for facility‑based hospice patients
- Provide on‑call support, including PRN and scheduled visits during evenings and weekends
- Communicate effectively with patients, families, facilities, and interdisciplinary team members
- Accurately document all patient care activities in the electronic medical record
- Support continuity of care while maintaining hospice philosophy and standards
- Active Registered Nurse (RN) license
- BLS/CPR certification
- Hospice nursing experience preferred
- Strong assessment, communication, and documentation skills
- Ability to handle difficult conversations and emotionally challenging situations
- Homecare Homebase (HCHB) experience
- Strong teamwork and collaboration skills
- Comfort working with end‑of‑life care, including complex family dynamics
- Ability to adapt to both routine and on‑call responsibilities
