Home Health Physical Therapist Assistant (PTA) - Contract
Location: Easton, MD: Patients’ homes within the branch service territory

We are seeking a dedicated and compassionate Home Health Physical Therapist Assistant (PTA) to provide high-quality rehabilitative care in patients’ homes. This role involves traveling to multiple patient residences each day within the assigned service area and working closely with a supervising Physical Therapist to support patient recovery and mobility goals.

- Days: Monday–Friday
- Hours: Typically 8:00 AM – 4:30 PM (varying based on patient schedules)
We are seeking a dedicated and compassionate Home Health Physical Therapist Assistant (PTA) to provide high-quality rehabilitative care in patients’ homes. This role involves traveling to multiple patient residences each day within the assigned service area and working closely with a supervising Physical Therapist to support patient recovery and mobility goals.
Key Responsibilities
- Provide physical therapy services to patients in their homes as directed by the supervising Physical Therapist
- Implement individualized treatment plans to improve mobility, strength, balance, and overall functional ability
- Assist patients with therapeutic exercises, gait training, and functional mobility activities
- Monitor patient progress and report observations, outcomes, and concerns to the Physical Therapist
- Document all treatments, patient responses, and progress accurately and timely per agency and regulatory requirements
- Educate patients and caregivers on exercises, safety precautions, and use of assistive devices
- Maintain a daily travel schedule and manage time effectively to meet patient care needs
- Adhere to infection control, safety standards, and home health best practices
- Current, active Physical Therapist Assistant (PTA) license
- Reliable transportation and willingness to travel daily within the service territory
- Ability to travel to multiple patient homes per shift
- Strong time management and organizational skills
- Effective verbal and written communication
- Ability to work independently in a home-based care environment
- Compassionate, patient-centered approach to care
- Navy blue scrubs or
- Navy polo shirt with khaki pants
- Yes — Daily travel required

Salary : $35 - $38
