Travel LPN – Home Health

Job Details

• Weekly Hour Guarantee: 32 hours
• Productivity Expectations: 30 points
• Coverage Area: Madison County
• Schedule: Monday–Friday, 8:00 AM–4:30 PM
• On‑Call / Weekend Requirement: One weekend per month
• Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last patient
• Pay Rate: $45-$46/hr
  
  

Position Summary

The Travel LPN will provide high‑quality home health care to patients within the Licensed Practical Nurse scope of practice. This role requires strong clinical skills, independence, and the ability to manage a field‑based caseload efficiently.

• Deliver patient care in accordance with LPN scope and home health regulations
• Perform assessments, treatments, and documentation accurately and timely
• Communicate effectively with the interdisciplinary care team
• Follow care plans and report changes in patient condition
• Maintain productivity expectations while ensuring quality care
• Travel throughout assigned coverage areas to complete patient visits
  
  

Required Qualifications

• Active LPN license in Alabama or multistate compact LPN license
• Home health experience
• Proficiency with HCHB (Homecare Homebase)
• Valid driver’s license and reliable transportation

• Scrubs: Black or blue
