Job Title: Case Manager RN

Location: Presque Isle, ME 04769

Duration: 4 Months (possibilities of extension)

Shift: 08:00 AM - 04:30 PM (Monday - Friday)

Job Responsibilities

• Conduct comprehensive admission and ongoing case management assessments, including medical necessity, level of care validation, and progression of care.
• Drive length of stay optimization and access day performance through proactive discharge planning and daily interdisciplinary collaboration.
• Ensure CMS compliant reviews and form securement, including timely admission reviews, continued stay determinations, IMM and MOON administration.
• Coordinate care transitions, post acute placement, to prevent avoidable delays and readmissions.
• Maintain accurate, timely clinical documentation supporting medical necessity, care progression, and regulatory requirements.
• Partner with physicians, nursing, social work, and ancillary teams to resolve barriers to discharge and align care plans.
• Support audit readiness and compliance.
• Monitor high risk populations and identify opportunities for early intervention to reduce excess days.
• Adhere to policies, standards, and CMS Conditions of Participation.
  
  

Required Qualifications

• Bachelor's Degree.
• RN license.
• Case Management experience.
• Patient facing/bedside presence experience.
• Cerner EHR experience.
• Acute hospital setting experience.
