Job Overview
We are seeking a compassionate and dedicated Community Integration Specialist to support individuals with diverse needs in their daily lives. This role involves providing personalized care, fostering community engagement, and promoting independence for residents in various care settings. The ideal candidate will have experience working with people with developmental disabilities, seniors, or individuals requiring specialized care such as Alzheimer’s or dementia care. Duties
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs, administering medications, and maintaining accurate care plans in compliance with HIPAA regulations
- Provide patient care that includes meal preparation, feeding assistance, and light housekeeping tasks such as cleaning and laundry
- Support residents during outings and community activities to promote social engagement and independence
- Utilize equipment such as Hoyer lifts and assist with heavy lifting to ensure safe transfers and mobility support
- Observe and document patient progress, behavioral changes, or health concerns for team review
- Follow established protocols for Alzheimer’s, dementia, memory care, autism spectrum disorder, and other specialized care needs
- Maintain a safe environment by adhering to safety procedures including CPR and first aid protocols
- Collaborate with healthcare professionals to implement individualized care plans tailored to each resident’s needs
Qualifications
- Prior experience in assisted living, home & community care, nursing homes, or long-term care settings preferred
- Knowledge of patient monitoring, medication administration, and vital signs measurement
- Certifications such as CPR and First Aid are highly desirable
- Experience working with people with developmental disabilities or special needs such as autism or Alzheimer’s is preferred
- Ability to perform heavy lifting and assist with mobility using equipment like Hoyer lifts
- Strong interpersonal skills with a compassionate approach to caregiving
- Knowledge of HIPAA regulations ensuring confidentiality of patient information
- Experience in social work or home health services is a plus
- Valid driver’s license may be required for community outings or appointments
