What You’ll Do:
• Nursing Care & Support: Provide nursing supports to individuals as needed, including direct care when necessary.
• Documentation & Medical Records: Complete nursing notes, including medical changes, interventions, recommendations, and communications with providers, day services, and families. Maintain appropriate medical records in accordance with OPWDD and best practice standards.
• Monitor Health & Medication: Continuously monitor individuals’ medical needs, provide treatments, and document medical histories. Ensure medications are being administered properly, and take appropriate action if there are discrepancies or errors.
• Medication Management: Ensure prescriptions are filled/refilled, record physician's medication orders, monitor medication administration sheets, report errors appropriately, and dispose of outdated medications according to policy.
• Clinical Data & Reports: Review clinical data and upload medical information into My Health and Evero. Provide weekly reports to the IDT (Interdisciplinary Team) and ensure accurate documentation.
• Infection Control & Preventive Measures: Implement protective and preventive measures regarding communicable diseases and promote health and hygiene practices.
•Collaboration: Work under the supervision of a Registered Nurse and collaborate with other healthcare providers to ensure the highest level of care for individuals.
• Healthcare Provider Management: Add and update healthcare provider information in systems to maintain accurate medical records.
Qualifications:
Educational Requirements
• High School Diploma
Must be a Licensed Practical Nurse (LPN) in New York State, licensed by the New York State Department of Education.
Experience & Skills:
• Preferred two years of experience working with individuals with intellectual disabilities, but not required.
• Ability to review and manage medical records, communicate effectively with individuals, families, and staff, and manage healthcare documentation.
• Certifications: AMAP, SCIP, and CPR certification (or ability to obtain within six months of hire).
• Strong organizational skills, ability to work independently, and a commitment to maintaining the highest standards of healthcare and safety.
