Brief Description

The Family Nurse Practitioner Resident insures delivery of comprehensive health care in accordance with the philosophy, standard of practice, objectives and policies of The Institute for Family Health. The Nurse Practitioner Resident is responsible for diagnosis and treatment of common and chronic illness focusing on health promotion and disease prevention activities including patient education and counseling; the FNP is cognizant of the philosophy, standards, objectives and policies of the Residency, the Department and the Organization.

ESSENTIAL JOB DUTIES

  • Participate in the FNP Residency providing primary care at clinical sites
  • Develop a panel of patients at primary clinical sites
  • Participate in the FNP Residency specialty rotations at assigned sites
  • Participate in Harlem Family Medicine didactics curriculum as scheduled.

  • Clinical/Technical/Service

Demonstrates the ability to perform clinical/ technical/ service/ administrative tasks:

  • Health Assessment, Treatment, and Maintenance
  • Demonstrates competent level of technical skills.
  • Conducts health assessment interviews.
  • Performs complete physical assessment.
  • Orders and interprets diagnostic tests.
  • Develops plan of care based on the diagnosis and individual client needs.
  • Prescribes medications and treatments, as appropriate.
  • Works collaboratively with the health care team and makes referrals to other disciplines, as indicated.
  • Records, reports, and interpret the client’s responses to plan of care.
  • Teaches, counsels and assists individuals and families to assume responsibility for prevention of illness and the promotion, maintenance and restoration of health.
  • Collaborates with other members of the team in the provision of regular health screening and other health promotion measures.
  • Identifies available community resources helpful to clients, refers and assists clients in utilizing such resources.
  • Makes home/hospital visits to clients and/ or takes calls, as appropriate.
  • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychological, educational, safety and related criteria, appropriate to the age of the patients served in assigned area.
  • Provide medphone coverage, as directed by supervisor.
  • Provide patient care at Phillips Family Practice on Saturdays during school vacation, as directed by supervisor.
  • Provides feedback to Nurse Manager, Practice Administrator, Program Director and/or Medical Director regarding policies and practices affecting the performance of the practice and/or their individual performance.
  • Organizes and administers the SBHC quality improvement activities.
  • Age Specific Competencies (all patient population served) Neonate/Infant, Pediatric, Adolescent, Adult, Geriatric.
  • Identifies physical, behavioral and emotional characteristics typical for the age

group.

  • Modifies approaches based on patient age-specific needs and responses to treatment.
  • Provides care for patients based on age-specific needs.
  • Uses communication techniques, which are age appropriate.

  • Is punctual and maintains a satisfactory attendance record.
  • Is appropriately attired for the office.

  • Observes the Health Care System’s compliance policies.
  • Demonstrates an awareness of the philosophy, standards, objectives and policies of the Institute and the FNP Residency Program.

  • Participates in special projects and committees.
  • Participates in Institute committees as directed by Medical Director and/or Program director.

  • Meets regulatory, licensure, and annual health assessment requirements.

  • Acts as a preceptor for selected students and staff.
  • Maintains collaborative practice agreement with physician as required by NYS law.
  • Avails self of educational opportunities to maintain/enhance competence and knowledge in advances of health care delivery.
  • Attends relevant workshops, conferences, and seminars.
  • Provides input to managers for performance evaluations of staff.
  • Determines own professional needs through ongoing self-assessment, peer and care team feedback and evaluation of role performance and assumes responsibility for meeting those needs.
  • Provides presentations and educational offerings.
  • Membership in professional and related organizations is encouraged.
  • Maintains membership in professional and related organizations, as encouraged.
  • Mentors and develops staff.
  • Serves as a role model and resource person for professional and paraprofessional staff.
  • Participates in Program Educational Activities such as Didactics, Case Conferences, Program Meetings, Specialty Rotations, Community Based Organization Rotations and program specific trainings
  • Meets the expectations outlined in the Program Guidelines for the NP Residency Program.
  • Communication/Relationships
  • Demonstrates a professional, courteous, and respectful attitude in dealing with patients, families and significant others.
  • Displays courtesy, tact and patience during interactions with all members of the hospital staff and extended community.
  • Demonstrates a professional, courteous, and respectful attitude in dealing with clinical practice staff.
  • Communicates department and hospital policy and procedures to staff.
  • Projects a professional image.
  • Reports to the Medical Director for the practice area.
  • Collaborates with community agencies to provide care to clients and groups in the community as approved by IFH.
  • Practices as a member of the interdisciplinary care team via effective and respectful communication and collaboration.
  • May be required to perform other duties as assigned.

EDUCATION

  • Bachelor’s degree and Master’s degree in Nursing required.
  • Graduate of an accredited Family Nurse Practitioner program (within 18 months of start date of the Residency) required.

LICENSURE/CERTIFICATION

  • Current NY State Registered License and Registration required.
  • Nurse Practitioner Certification and Registration required.
  • Current Practice Agreement required.
  • Certified or eligible for certification by recognized specialty organization required.

COMPUTER PROFICIENCY

  • Basic computer and internet navigational skills Required.
  • Computer literacy with Windows-based operating systems and MS Office applications (Word, Excel, Outlook, Powerpoint) Required.
  • Familiarity with electronic health records (EHR) technology Required.

Linguistic Skills

  • Bilingual, Spanish preferred
