We are currently seeking an Integrated Behavioral Health Provider who is mission-driven, is energized by teamwork, values relationships, and is passionate about brief and impactful interventions.
Key Responsibilities
- Provide brief, solution-focused therapeutic interventions to support patient well-being.
- Respond to same-day warm handoffs from medical and behavioral health providers.
- Conduct concise, goal-oriented assessments to inform care planning.
- Support patients experiencing depression, anxiety, chronic illness, and psychosocial stressors.
- Collaborate closely with a multidisciplinary care team to coordinate comprehensive care.
- Document patient interactions and clinical notes efficiently and with clinical relevance.
- Co-lead care planning and population health initiatives for patients with complex or high-risk needs.
- Licensed behavioral health clinician in the state of Ohio (LSW, LISW, LPC, LPCC, MFT, or IMFT)
- Experience delivering brief, evidence-based interventions (e.g., CBT, ACT, solution-focused therapy)
- Comfortable working collaboratively with medical providers in an integrated care setting
- Strong, confident communicator who thrives in a team-based environment
- Experience working with ethnically and culturally diverse individuals and families
- Bilingual in Spanish (verbal and written proficiency)
